The employer's name should be entered by the applicant.
Note: Occupational data are principally used for statistical purposes. This information, along with information obtained from Items 10 (certificates held), 11 (occupation), 14 (total pilot time), and 15 (pilot time in last 6 months), may be important in determining whether a SODA may be issued, if applicable.
