07/24/2000: Please remember: all exams, regardless of certification status, should be electronically submitted using the DIWS system. Whether the certificate has been issued, deferred, denied, whatever the case may be, you must electronically submit the exam.

05/31/2000: **Ammendment (02/20/2003)**It is no longer necessary to mail supporting documentation separate from the hard copy.** The FAA Original Copy of the 8500-8 form must be mailed to the Aerospace Medical Certificiation Division (AMCD) after it has been electronically submitted. Any additional information that applies to a transmitted exam should be mailed separately. The Cover Sheet that is now provided online in the SUPPORT SECTION should be used for all additional information. Failure to use this cover sheet will delay the processing of the airman's data. Additional Information should be sent in separately from exams, with one exception: If you are deferring an airman please attach the exam to the data and mail in immediately.

Please indicate the Certificate Status on all hard copy exams by checking the appropriate box under item 62. The rest of the backside of the form can remain blank. Checking item 62 is a temporary step needed to assist the Certification Division in double checking the system's performance and accuracy. You will be notified when this step is no longer required.
SPECIAL ISSUANCE NOTICE: When issuing a "Time Limited" Certificate, please enter the actual date of expiration of the medical certificate under item 60.

02/11/2000: It has become apparent there is strong need to clarify policy concerning the mailing in of additional information: This notice stands as the proper procedure, per Dr. Warren Silberman, and is to be followed by all Aviation Medical Examiners.

The Cover Sheet that is now provided online in the SUPPORT SECTION should be used for all additional information that is mailed to the Certification Division regarding a transmitted exam. Failure to use this cover sheet will delay the processing of the airman's data. The cover sheet will help the Certification Division process the data more efficiently.

01/11/2000: The AMCS/DIWS system is a "one way" data entry system only. You can not "view" previous information or go back to check a recently submitted exam for errors. If an airman selection does not show a "P" in the status box, do not pull that airman up, unless you intend to start a new exam on that airman.

Should you discover an error after you have submitted an exam, please fax or e-mail the AMCS Hotline and let us know what needs to be corrected and the exam will be corrected by the Certification Division. Please be sure to include the Airman Name, the MID#, the Applicant ID#, the Certificate FF# and the AME# in your email or fax to help us reference the airman in question.
